Human error assessment and reduction technique (HEART) is a technique used in the field of human reliability assessment (HRA), for the purposes of evaluating the probability of a human error occurring throughout the completion of a specific task. From such analyses measures can then be taken to reduce the likelihood of errors occurring within a system and therefore lead to an improvement in the overall levels of safety. There exist three primary reasons for conducting an HRA; error identification, error quantification and error reduction. As there exist a number of techniques used for such purposes, they can be split into one of two classifications; first generation techniques and second generation techniques. First generation techniques work on the basis of the simple dichotomy of 'fits/doesn't fit' in the matching of the error situation in context with related error identification and quantification and second generation techniques are more theory based in their assessment and quantification of errors. HRA techniques have been utilised in a range of industries including healthcare, engineering, nuclear, transportation and business sector; each technique has varying uses within different disciplines. HEART method is based upon the principle that every time a task is performed there is a possibility of failure and that the probability of this is affected by one or more Error Producing Conditions (EPCs) – for instance: distraction, tiredness, cramped conditions etc. – to varying degrees. Factors which have a significant effect on performance are of greatest interest. These conditions can then be applied to a "best-case-scenario" estimate of the failure probability under ideal conditions to then obtain a final error chance. This figure assists in communication of error chances with the wider risk analysis or safety case. By forcing consideration of the EPCs potentially affecting a given procedure, HEART also has the indirect effect of providing a range of suggestions as to how the reliability may therefore be improved (from an ergonomic standpoint) and hence minimising risk.

The Compilation of Final Fantasy VII is a metaseries produced by Square Enix. A subseries stemming from the main Final Fantasy series, it is a collection of video games, animated features and short stories based in the world and continuity of Final Fantasy VII (1997). Officially announced in 2003 with the reveal of Final Fantasy VII: Advent Children, the series' core products are four video games and one movie release. Alongside these are tie-in products and spin-offs including books, mobile games and an original video animation. Advent Children and the mobile title Before Crisis: Final Fantasy VII are a sequel and prequel to VII, respectively focusing on Cloud Strife, the original game's main protagonist, and covert operatives known as the Turks. Crisis Core: Final Fantasy VII follows Zack Fair, a major character in VII, while Dirge of Cerberus: Final Fantasy VII, a sequel to Advent Children, follows Vincent Valentine, one of the original's optional characters. The series was conceived by Yoshinori Kitase, the original game's director, and Tetsuya Nomura, the main character designer. Nomura would become the main designer for each entry in the Compilation. Other returning staff include writer Kazushige Nojima, art director Yusuke Naora, and composer Nobuo Uematsu. The video games belong to different genres, with none of them being traditional role-playing games due to production pressures related to the genre. While the first title revealed was Advent Children, it ran into delays during post-production, so the first Compilation title to be released was the mobile game Before Crisis. Of the core titles, Before Crisis is the only one still unreleased in the west due to issues with overseas platform compatibility and staff changes. Reception of titles in the Compilation has been mixed, with Advent Children being praised for its visuals and criticized for its confusing nature. Before Crisis and Crisis Core have received praise, while Dirge of Cerberus garnered a mixed response. The presentation of the Compilation as a whole has met with a mixed response, and later staff linked it to the decline of the Final Fantasy series' prestige in the West. The Compilation inspired the creation of Fabula Nova Crystallis Final Fantasy, a similar subseries of linked video games.

MOOSE, originally an acronym for Man Out Of Space Easiest but later changed to the more professional-sounding Manned Orbital Operations Safety Equipment, was a proposed emergency "bail-out" system capable of bringing a single astronaut safely down from Earth orbit to the planet's surface.The design was proposed by General Electric in the early 1960s. The system was quite compact, weighing 200 lb (91 kg) and fitting inside a suitcase-sized container. It consisted of a small twin-nozzle rocket motor sufficient to deorbit the astronaut, a PET film bag 6 ft (1.8 m) long with a flexible 0.25 in (6.4 mm) ablative heat shield on the back, two pressurized canisters to fill it with polyurethane foam, a parachute, radio equipment and a survival kit.The astronaut would leave the vehicle in a space suit, climb inside the plastic bag, and then fill it with foam. The bag had the shape of a blunt cone, with the astronaut embedded in its base facing outward. The rocket pack would protrude from the bag and be used to slow the astronaut's orbital speed enough so that they would reenter Earth's atmosphere, and the foam-filled bag would act as insulation during the subsequent aerobraking. Finally, once the astronaut had descended to 30,000 ft (9.1 km) where the air was sufficiently dense, the parachute would automatically deploy and slow the astronaut's fall to 17 mph (7.6 m/s). The foam heat shield would serve a final role as cushioning when the astronaut touched down and as a flotation device should they land on water. The radio beacon would guide rescuers.General Electric performed preliminary testing on some of the components of the MOOSE system, including flying samples of heat shield material on a Mercury mission, inflating a foam-filled bag with a human subject embedded inside, and test-dropping dummies in MOOSE foam shields short distances. U.S. Air Force Capt. Joe Kittinger's historic freefall from a balloon at 103,000 ft (31,000 m) in August 1960 also helped demonstrate the feasibility of such extreme parachuting. However, the MOOSE system was nonetheless always intended as an extreme emergency measure when no other option for returning an astronaut to Earth existed; falling from orbit protected by nothing more than a spacesuit and a bag of foam was unlikely to ever become a particularly safe—or enticing—maneuver.Neither NASA nor the U.S. Air Force expressed an interest in the MOOSE system, and so by the end of the 1960s the program had been quietly shelved.
